Licensed Independent Social Worker (LISW)Home Health & Mental Health | VNA of Ohio | Cleveland, OH

VNA of Ohio is hiring a mission-driven LISW to support both our Home Health and Community Mental Health programs. You'll provide in-home psychosocial assessments, short-term counseling, and care coordination for patients recovering from illness or managing chronic mental health conditions.

What You'll Do:

• In-home assessments and brief therapy for home health patients

• Ongoing mental health therapy for adult clients (Medicaid)

• Collaborate with nurses, therapists, and case managers

• Support transitions, resources, and whole-person care

What We’re Looking For:

• Active Ohio LISW (LISW-S a plus)

• 2+ years clinical experience (home-based or CMH preferred)

• Strong clinical judgment, documentation, and empathy
